Good shock value should make the reader feel like, "D*mt it! He did that crazy thing?! I totally missed the hint in the first chapter."
Not like, "Why did he suddenly do this crazy thing?! There was no hint, he was always portrayed as kind."

A Song of Fire and Ice is a famous example, and one reason why this fiction is hated.
