The Female Lead hate the Male Lead, but he keep pursuing her since he thinks she is just being Tsundere. If the FL marry the ML, she will die, so she tries to stay away from him as possible. But it doesn't work and the FL is now best friends with anxiety and depression. The ML notice her unease, but fails to realize the cause. He tries to comfort her by being more affectionate than usual. It has the opposite effect.

1. You'd think the FL has friends.

2. You'd think that if the ML cared, he'd notice that he's the cause and try to find out why via her friends.

From here on out, you can have multiple outcomes: ml finds out real reason = starts distancing himself as he believes her.
ML learns why, laughs and shrugs it off until stuff becomes horrible.
ML learns why, can't believe it, is more than slightly insulted that she's using such an 'unrealistic reason' and their relationship grows distant, then hateful.
ML decides to try being heroic, looks for reason why she would die by marrying him.
FL gathers courage to approach ML and be honest with him; another possible divergent.
