heart attack after witnessing a tractor.

I've also seen 'Playing video games', 'Answering weird online surveys/What type are you questionnaires', 'Wishing for it', 'Going to Egypt/old mystical place/Fairy Ring', 'Drugs', 'Falling off the world', 'Abducted by lightning/tornado', 'the Earth was destroyed to make room for a highway' 'fell off a busted balcony', 'bus accident', 'exploded classroom', 'hole in the time-space continuum', and 'walking down the street on halloween'.

Honestly though, Isekai isn't about how you got there. It's about what happens afterwards. Skipping the death entirely is probably the best way to go.

We all know and love truck-kun but I feel like the variety of deaths that cause an isekai are pretty humdrum: car accident, stabbing by some insane jerk, tripping and falling down stairs, and terminally ill patient finally passing are some that immediately spring to my mind and I'm just wondering where's the pizazz? What's the most unique or unusual way you've ever read someone get transported into another world?
Fell from train, Got shot while chasing criminals, Choked on moldy bun while cursing Novel, Fell Victim to Fire accident, Suicide, Got buried Alive, Crushed by chandelier while receiving award, Burnt into a crisp by lightning, Got locked into his own future royal coffin, There are wayyyy more if you have read enough.

I haven’t really read any unique ones. I once wrote one when she first got her bones broken by a crowbar and then shot by a policeman. Creepy sentient space brought her there through a ritual.

Although dodging truck-kun, ducking down an alley where truck-kun impossibly squeezes through, and then getting high up on a gate only to still get hit by truck-kun driving vertically was pretty funny. Unfortunately the writing of that story was trash.

In my book it happens when a portal opens because someone agreed to something a flamingo said while taking a shower.
Something involving a scam caller could be fun.
Loosing to seto kaiba in a duel.

I feel like in older Isekai like Fushigi Yuugi, The Wizard of Oz, and The Lion, The Witch and the Wardrobe it was less... codified, how one crossed over?

These days it feels like it's mostly variations on ways to die... maybe to imply it's a permanent thing unlike the older isekai, where people usually returned to the "normal" world sometimes?

Extremely hot take: maybe escapism has become way more hardcore, that the thing to daydream about is leaving the world and never coming back, rather than visiting another world for a while and coming back and then visiting the other world again.
Escapism has become a noticable thing for Reader and Writer alike since ancient times.

Werewolf stories were like that, a supposedly "average teenage beauty" met handsome hidden werewolf or vampire and then they date or something.

It's just nowadays trend dictate that they straight up turning shitty Self-Insert into game/novel/comic/anime fanfiction genre and upgrading them into Full-fledged bonafide isekai story.

Those are common but: Suddenly opening portal, suddenly opening portal in pc (or other video console), sudden invitation of good leading to suddenly opening portal. Dying during playing hero/innocent bystander victim.
Oh, I just read here on SH a start where a girl gets her head cracked against the wall by her mother because she was impolite. She died apologizing to her for being such a bad daughter. That was... new...
