I have read many stories, and I have noticed that almost no author inserts images in the middle of the story with "insert/edit image."

I don't see anything wrong with them not doing it because I don't do it either. I'm just a bit curious about why almost no one, uses this resource, especially in anime or video game fanfics, to provide a clearer visual context of the characters' appearance for those who read the fanfic without knowing anything about the original series.

I would only do it if it adds to the story, but in novel format, the reader must imagine the scene themselves. The more visual mediums, the less you have to imagine visually. The beauty of the novel medium is that there is a lot of visual ambiguity. A blue dress will look different to everyone. Even the shade of blue might be different in their head.

I've got chapter pics, but I'm a bit of a traditionalist with inserting them at the beginning of each chapter.

I do so often. Not every story will have a need for image input though. Admittedly it isn't a common feature, but still a wanted one.

To prevent the images from breaking, you need to look for websites that do not have a time limit for uploading images or that do not send you a link with tens of thousands of characters. The method I use is to use a mobile application called QuickIB, which allows you to upload images permanently and with a very short link.
