Well, that's certainly an inside joke. The comedy of it was to always make sure someone didn't get too lazy or complacent about anything in a work that required careful consideration.

We used the word "carefully" because it's a smart way of making sure your audience knows you've thought about what you're doing, but in a way that avoids falling into "word salad" that feels too sloppily metaphorical or any such bad analogies.

I'm not sure, but I think I was either first or one of the first who started spamming it. K5Rakitan would use it as an answer to pretty much every question that couldn't be answered with 'don't forget to wear a condom.' It irked me, and I began hating the word. I started spamming it everywhere so that people other than me would hate it, and I kinda achieved this goal.
