Obviously, you can omit the volume part if there no volumes. Generally, it's nicer to see both title and a chapter number.

And that's the best way as long as the title does not give away too much about the content of the chapter.

Hmmm...for me sometimes such characters feel shallow because often they feel unreal. They are damaged but they do not feel realistic, "damaged" just seems just another badge to make a character seem cool.
