well I think what most people look for in a fanfic is the same feelings they felt with the original source, at least that's for me
like crying when Jiraya died, clenching our assholes when Madara went against a gazillion ninjas, cumming when Guy beat the shit out of Madara, etc.

I'm not sure myself, so if someone can correct me, do that. But I assume that not every type of psychopath has impaired empathy. For example, it can manifest in seeing thing, not knowing what is real and what is not.
