why not turn the hand around and have the stem coming from his veins below the palm

I really like the idea going on here. Imo the position of the stem coming out from the side is a little awkward and the arm pose of the first version makes a better composition compared to the new version.

If you want to keep the new version, then I'd move the arm up a bit, make the fruit bigger and set the bird lower to make better use of the space and apply the rule of thirds.

Because I think that the back of the hand fits better, and is easier to draw.

If you don't mind:

The thing is, I think what you need to worry more about is the shape of your hand, rather than lighting or rendering. As of now, it is hard to make out if it's a hand or something else. Clarity first. Considering that this is a book cover, you'll have to think about how it would look zoomed out as well. Make the hand larger. Also, what I learned about composition is that horizontal placement equals calmness, and it doesn't spark interest. Make it diagonal, swirl, or whatever.

One last thing, a general tip when drawing is to use long and confident strokes to avoid wobbling lines.
