Nothing. Slice of life, slow burn romance is always best.
There's gotta be some sort of conflict driving a story, even in slice of life. Zero conflict means nothing is happening.

Personally I like external conflicts like bad home life, bullying, insecurity, medical conditions, etc causing problems, and the two romantic partners finding solace in each other. hurt/comfort destroys me

"Lacking communication" — this one is always frustrating to read, but it's also a necessary evil in many stories. It can open up some juicy drama plots, as long as it's done in a reasonable manner (and it is very much reasonable for people to communicate poorly; that's just accurate to life and romance)

Hate love triangles though. Irredeemable

I hate both. Love triangles suck, and lack of communication is stupid.

I prefer “Us against the world.” That is the best.

Love triangles are fine if they are right triangles. Obtuse triangles are the ones that suck, because of the lack of communication that makes them obtuse. I'd like to see more acute love triangles because I like acute things.

As a person who wants to see the consequences of having multiple love interests, anime series such as White Album 2 and Rumbling Hearts is one of my favorites even though it was a hard watch. After all, watching characters as they struggle through their choices is one of the best thing I would want in a romance.

Oh yeah, I definitely love School Days too, the ending was hilarious.

I wouldn't know what the best conflict is, but here are some alternatives that readers might relate to:
MC relationship vs. the world; these are when family or other factions try to break the relationship apart. It can also be when culture or societal norms get in the way of the relationship.
MC vs. self; like a person so insecure that they try to speedrun the relationship into breaking up, or when a person is so scared that they'll struggle to act on their feelings.
Another conflict I've seen is when there is a physical limitation, like a queen falling in love with her eunuch servant, or when an asexual person falls for a nympho.
Another conflict is power imbalance; how will the poor lover deal with being thrust into a world of merchants or politicians? Dating your boss might not be easy.

Age difference.

I know it gets a lot of sus-flak, but it is most certainly a potential conflict, often not directly for the couple, rather for those perceiving them or who have some involvement/sway towards them. I would be personally interested if a romance story existed with say, 100+ years of age/experience difference, with focus to realistic interaction (not to exist for the mere sake of a trope).

That's reserved only for novels with a female demographic. Gotta look out for the bois who want wholesome vanilla romance.
Like jesus every time my mom is listening to a romance audiobook it involves cucking or a love triangle or some other extremely convoluted bullshit.
